  • Gateway provides services at our facilities, in-home or in the community in the 5 boroughs of New York City and Westchester County.

  • In order to receive OPWDD services at Gateway, you must have OPWDD eligibility.

    Each program has unique eligibility requirements. More information can be found on the program information page here.

  • Those who qualify for OPWDD services will have assistance coordinating their healthcare with a Care Coordination Organization (CCO).

    CCOs provide comprehensive care management, care coordination and health promotion, comprehensive transitional care from inpatient to other settings, individual and family support, referrals and links to services. The CCOs in Gateway’s region are Advance Care Alliance, Care Design NY, Tri-County Care and Partners Health Plan FIDA-IDD. In addition to those above, Westchester County residents can also use LifePlan CCO.

  • During the enrollment process, Gateway will work with the designated Care Manager who will submit a psychological evaluation, medical exam, Lifeplan and Level of Care. You may already have these documents from your enrollment into OPWDD or other programs.

    If you need assistance obtaining these documents, Gateway will work with your Care Manager to get you started!

  • Yes, we do!

  • Gateway has bilingual staff who are able to speak English, Spanish, French, Mandarin, Cantonese, Korean, Russian, Bangla/Bengali, Tagalog and more. We also have access to interpreters, when needed.

  • If you are enrolled in a Gateway program, you will receive transportation to and from our services or a complimentary MetroCard if you wish to travel on your own. Nutritious meals are also provided throughout services, free of charge. Those receiving telehealth services will be provided with a tablet or other materials if needed to connect and engage with us!

  • Yes! Gateway has other services too! Please see below for more information on our mental health Continuing Day Treatment Program and our Non-Emergency Medical Transportation service.

    We have a program focusing on mental health under the auspices of the Office of Mental Health, known as our Continuing Day Treatment Program (CDTP). Our CDTP aims to provide comprehensive mental health services that guide individuals towards better mental health management and overall stabilization in addition to medication management.

    Gateway has also opened Prevocational services and programming to OPWDD enrolled individuals. Please use our enrollment form here to enroll or find out more information.
